To Apply for Auditing Services, complete the Application for Service - LP-109 form. Clients must submit an application for service which includes a completed LP Form 109 - Application for Service, a cover letter, and a complete copy of the client’s program documentation. The Quality Assessment Division provides billing account information.

When requesting QSVP services for the first time using the LP Form 109, the client must

  • Enter applicant information including name, billing address, physical address, tax id number, telephone number, fax number, and email address;
  • Identify the type of Quality Assessment Division Service requested;
  • Enter the FSIS establishment number if applicable;
  • Identify the type of operations conducted;
  • Identify the legal status of the operation;
  • Identify the financial interest of the applicant;
  • Agree by checkmark to the 4 requirements stated on the right;
  • Sign and date the Form.

The completed LP Form 109 must be sent to the Quality Assessment Division along with the cover letter requesting service and the program documentation. These documents can be sent via email to QAD.BusinessOps@usda.gov or to the address below.

If any information on the original LP Form 109 changes, then the client must submit a revised, updated LP Form 109.

Eggs

Clients requesting Export Certification Services for the first time, or requesting services for new countries and/or products must complete the application worksheet. The client must complete the following:

  • Enter applicant information including name, billing address, physical address, tax id number, telephone number, fax number, and email address;
  • Identify the type of products to include the common name of the product(s)
  • Identify the Country(s) that the client wishes to export product
  • Sign and date the worksheet
  • Completed documents for requests will be returned to the applicant.

Forms

All the information needs to be completed on a single sheet in the format provided. Worksheets not properly completed will not be processed. The final product being exported must be manufactured in a facility listed on the Federal list, the name, location and plant number must match the information listed including ID numbers.
